Inlays and Onlays

When a patient’s molars or premolars are too damaged to support fillings, but not damaged enough to require crowns, cosmetic dentists frequently recommend inlays or onlays to restore the chewing surface of the teeth. An inlay fits within the grooves between the cusps on the chewing surface of a tooth, whereas an onlay wraps up and over the cusp tip, so it covers more of the tooth's surface.

Inlays and onlays may be used to stop the spread of bacteria and preserve the integrity of natural teeth. This method of restoration also makes it possible to replace outdated dental work and old unsightly silver fillings with beautiful tooth-colored restorations, designed perfectly to match the natural teeth.

Since inlays and onlays are prepared outside the patient’s mouth and then cemented or bonded to the tooth, they require two appointments to complete the procedure. During the first appointment local anesthesia is injected into the patient’s mouth to numb the area to be treated. A part of the tooth that is damaged is then carefully removed and the remaining portion of the tooth is shaped so that it can hold the inlay or onlay. Using a soft, clay-like substance, an impression is taken of the damaged tooth area and a mold is sent to a dental laboratory, where the custom inlay or onlay is created. A patient is fitted with a temporary sealant while waiting for the permanent inlay or onlay to be completed. At the second appointment, the temporary sealant is removed and the inlay or onlay is placed on the tooth and is adjusted for a precise fit. If the fit is satisfactory, the inlay or onlay will be cemented permanently to the tooth with a strong resin and polished to a smooth finish.

Inlays and onlays are designed for durability and are able to withstand everyday wear and tear. As a result, they can last from 10 to 30 years. Materials such as gold, composite resin or ceramics may be used to create inlays and onlays. Which material is chosen may be influenced by aesthetic appeal, strength, durability and cost.
